  • Step 2: Enter your name, mobile number and PIN code and click View Prices. Next, enter your DOB and select your plan type.
  • Step 3: Next, select your sum insured limit, policy add-ons (if required), and the policy term option. Click Next.
  • Step 4: Enter your KYC details and pay online to receive your policy via email. You can also download your Health Gain 3.0 plan onto your phone via Reliance Self-i.

How to File a Claim Under the Reliance Health Gain Policy?

Cashless Claims Process

  • Step 1: Visit a network or non-network hospital to get treated. You can use our hospital locator tool to find your closest Reliance network hospital for more streamlined claims.
  • Step 2: In case of planned treatments, inform us at least 48 hours prior to your admission by submitting a Pre-Authorisation form at the hospital TPA desk.
  • Step 3: The hospital TPA desk will then contact us to arrange your cashless treatment. We will provide confirmation on your preauthorisation request within an hour.
  • Step 4: For your final authorisation request, for discharge, we will give our confirmation to the hospital within 3 hours.
  • Step 5: In case your cashless claim application is rejected, you can still pay first and then apply for a reimbursement claim later.

Reimbursement Claims Process

  • Step 1: Visit any hospital of your choice to get treated, pay and keep all reports, receipts, prescriptions and bills.
  • Step 2: Contact us on 022 4890 3009 (Paid No.) and relay your claim details. You can also submit your claim on our website or through the Reliance Self-i app.
  • Step 3: Submit the claims form and all other relevant documents and bills to the Health Care Unit. This needs to be done within 15 days of your discharge.
  • Step 4: Health Care Unit team will review and verify your claim application and submitted forms. We may also request additional documents.
  • Step 5: Upon approval of your claim, we will transfer the payment amount to your bank account via NEFT.

Documents Required for Health Insurance Claims

  • Health claim form (all pages) duly filled & signed by the policyholder.
  • Copy of Photo ID proof of the Insured or nominee - PAN, Driving Licence, Passport Copy, Ration Card, etc.
  • Original CTS 2010 complaint: cancelled cheque with the printed name and account number of the policyholder.
  • All original medical reports, bills, receipts, discharge summaries and prescriptions.
  • Attested copy of First Information Report (In case of fatality & Permanent Total Disability)
  • Attested copy of Post Mortem Report and Death certificate (If required)

What Do I Need to Consider When Filing a Claim Under Reliance Health Gain 3.0?

Waiting Periods

  • Initial waiting Period: 30 days for all illnesses (not applicable for continuous renewal or accidents)
  • Covid-19 Waiting Period: 15 Days Waiting Period for Covid-19
  • Waiting Period for Mother and Child Care: For 1 and 2-year policy tenure: 24 months; For 3 years and above policy tenure: Zero waiting period
  • Specific Waiting Periods: 24 months for 46 diseases/procedures
  • Pre-Existing Diseases: Covered after 36 months
  • Chronic Shield Waiting Period: Waived off/Day 1 coverage.

Terms and Conditions

  • Misrepresentation, fraudulent or false statements and non-disclosure of any information will result in the cancellation of your policy.
  • If you have multiple health policies, you can choose to file a claim under any one of them.
  • The policy shall terminate if it is not renewed at the end of the policy period. A 30-day grace period will be provided to renew the policy.
  • There will be no coverage during the grace period. The only exception to this is when premiums are paid in installments, in that case, you will receive policy coverage.
  • In the event of any issues or complaints, please contact the Grievance Redressal Officer using the details provided in the policy document.

Policyholder/Insured Person's Responsibility

  • Always check the applicability of our cashless service with the hospital.
  • Undergo a medical examination by our medical practitioner, if needed.
  • Allow our medical practitioner to verify your medical records.
  • Submit your documents within the stipulated time for the claim.
  • Cooperate with our representatives to validate your claim.

  • You can transfer your existing health policy to Reliance General Insurance by using the health insurance portability option. Contact us to initiate the porting process at least 45 days before your renewal date for a seamless transition.

  • Pre- and post-hospitalisation expenses refer to medical costs incurred before and after hospitalisation.

  • A consumables cover will pay for single-use items, such as bandages, needles, gloves, and syringes, that are required for surgery and medical care. These aren’t generally covered under your base policy and can increase your overall out-of-pocket expenses. So, having the cover can be beneficial.

  • Typically, your health insurance policy is valid for 12 months, starting from the date it was issued. With the Reliance Health Gain 3.0, you can buy a multi-year plan up to 5 years
